Make an Appointment

Please take a moment to request an appointment and a member of our staff will be in touch with you shortly to schedule and confirm your appointment.

When we contact you to schedule your appointment, we will ask for your vision and medical insurance information. Please have the name of your insurance provider, your member ID and your group number (if applicable) for both insurances.  If you are not the primary policy holder, please also have the following information for the primary policy holder available: name, date of birth, address, and the last four digits of their social security number.
